Reservations
You can come in without making an appointment, but we give priority to appointments, so you may have to wait. If possible, please make an appointment at least one day in advance.
1) Reservation
Please make a reservation by phone or at the reception desk at least the day before (during clinic hours and on the day of your appointment).
What to bring
・Health insurance card
・Medication notebook (if you don't have one, your medicines, eye drops, etc.)
・Medical notebook *Only for those who have one
*Those who are currently receiving treatment at a clinic or hospital for high blood pressure, diabetes, etc.
If you have previously been prescribed eye drops or other medications at another clinic, please bring them with you.
2)受付
受付にある「診療申込書」に必要事項を記入の上、「保険証」、「紹介状」(検査結果、フィルム等)を添えて ご提出ください。
For returning patients, we will check your insurance card at the beginning of the month.
3)問診
スタッフが問診票の内容に基づき、症状についてお話をうかがいます。診療を行うための必要な情報(既往症や今使っている薬、アレルギーの有無など)について、わかる範囲で結構ですのでお答えください。治療に関するご要望などもお聞きしますので、遠慮なくご相談ください。
現在の眼の症状や病気の診断結果をもとに、治療方針を詳しく説明いたします。
5)お薬の処方・会計
診察後は待合室にてお待ちください。診療費の計算が終わりましたら、受付スタッフがお呼びいたしますので、受付へお越しください。
4) Examinations and consultations
Tests will be carried out according to symptoms and illnesses. These tests will be carried out by testing staff.
Based on these test results, along with the doctor's other tests, interviews, and examinations, the doctor will present the test results to the patient and their family.
We provide medications on-site, and we will issue prescriptions for those who would like generic drugs or pick them up at a pharmacy.

When you visit our clinic, please wear a mask and disinfect your hands at the entrance to prevent infection.
